A 58-year-old man has had palpitations for 6 hours. He feels faint and has central chest tightness. His pulse is irregularly irregular at 160/min and his blood pressure is 82/50 mmHg.

What is the most appropriate next step?

  1. ABisoprolol, review in a week

    Why not? He is unstable now, and a beta-blocker could lower his blood pressure further.

  2. BApixaban and routine cardiology referral

    Why not? Anticoagulation matters later, but instability is the immediate danger.

  3. CEmergency transfer to hospital by ambulance

    Best answer. AF with life-threatening instability needs emergency hospital care.

  4. D24-hour Holter monitor

    Why not? The diagnosis is already clear, and he is unstable.

  5. EReassure: new AF usually settles in 48 hours

    Why not? Some new AF does settle, but never assume that in an unstable patient.
